Remote follow-up coordination
We help arrange agreed follow-up appointments or remote consultations with your treating provider after you return home.
Local-care handover
With your consent, we help organise the transfer of records to your local clinician so your ongoing care stays connected.
A note on clinical responsibility: All discharge instructions, prescriptions and follow-up plans are determined by your treating provider. Vedam Care coordinates the non-clinical logistics and helps ensure information reaches you and your local clinician with your consent.
